Comparison of Mixed Allograft to Autograft in Multi Segment Lumbar Posterior-lateral Fusion

Abstract
Purpose: This study aims to find out compatibility of the mixed allobone graft in more thanthree-segment posterior lateral lumbar fusion, through the comparison of the bone union rate offrozen allobone graft mixed with local autobone with that of autobone graft harvested from iliaccrest.

Materials and Methods: The subjects of this study were total 25 patients who had more thanthree- segment posterior decompression, fixation, and posterior lateral fusion due to spinal stenosis.They had been followed up at least two consecutive years. Evaluation on the rate of boneunion was done by plain radiologic findings using Lenke¡¯s fusion scale at each 3, 6, 9 monthsand 1 and 2 years after the surgery. The difference of bone union rate was also analyzed betweenthe two groups classified by the type of bone graft. Statistically Chi-Square test was used.

Results: At early stage of follow-up period, there was no statistical difference in bone unionrate between the two groups, but at 6, 9 months, 1year and 2 year after the surgery, mean value ofLenke¡¯s fusion scale of mixed allobone graft was 3.25¡¾0.45, 2.75¡¾0.45, 2.17¡¾0.58, and 2.08¡¾0.67, respectively and that of autobone graft was 2.69¡¾0.63, 1.69¡¾0.48, 1.46¡¾0.52, and1.38¡¾0.51, respectively, which shows statistical difference between the two groups (p<0.05).

Conclusion: It is supposed that for more than three- segment posterior lateral lumbar fusion,autobone graft can be the first choice, as the allobone graft mixed with local autobone achievedlower rate of bone union and took longer time to reach bone union. But in case of shortage ofautobone, use of the mixed allobone graft can be considered as an adjuvant method.
